Orthodontics as an adult: the costs you can expect

Unlike for children and teenagers, German statutory health insurance does not pay for orthodontic treatment of adults – except in severe, medically justified cases that also require jaw surgery.

Treatment usually takes between 18 and 36 months. On top of that come regular check-ups, adjustments and possible additional treatments. Costs rise particularly with combined treatments, complications, premium materials or several treatment phases.

Typical costs for adults without insurance
TreatmentCosts
Fixed braces (metal brackets)€3,500 – 6,000
Ceramic brackets (tooth-coloured)€4,500 – 7,500
Lingual braces (invisible, inside)€8,000 – 12,000
Invisalign & other aligners€3,500 – 8,000
Retainer (stabilisation)€300 – 800

What does German statutory health insurance pay?

The sobering answer: for adults, practically nothing. Statutory health insurance only covers orthodontics up to the age of 18 – and only for severe misalignments in KIG groups 3 to 5.

The only exception are extremely severe jaw misalignments that require combined orthodontic and surgical treatment – for example when chewing, speaking or breathing is significantly impaired and a specialist report confirms the need. Even then, statutory insurance only covers the basic treatment; aesthetic extras such as invisible brackets or Invisalign are paid by you.

The result: the vast majority of adult orthodontic patients pay the full costs themselves – unless they have taken out supplementary dental insurance in advance.

What orthodontic dental insurance covers

Good supplementary dental insurance closes the gap that statutory health insurance leaves for adults. Modern plans cover practically all common treatments:

  • Fixed braces (metal and ceramic brackets)
  • Invisalign and other clear aligners
  • Lingual braces (invisible braces on the inside)
  • Retainers to stabilise after treatment
  • Functional diagnostic examinations
  • Check-ups and adjustments

Important: treatment that is already ongoing, diagnosed or planned is usually excluded from cover. Waiting periods and benefit limits in the first years also often apply.

FAQ

Frequently asked questions about orthodontics for adults

Why do adults need orthodontic insurance?

German statutory health insurance pays practically nothing for orthodontic treatment of adults. Braces or Invisalign cost between €3,500 and €12,000 – without insurance you pay the full amount yourself. A suitable plan can cover up to 100 % of these costs.

Which orthodontic treatments are covered?

Modern plans cover fixed braces (metal and ceramic brackets), Invisalign and other clear aligners, lingual braces, retainers, functional diagnostics as well as check-ups and adjustments.

What about waiting periods?

Waiting periods range from immediate cover to several months, depending on the insurer. Misalignments that have already been diagnosed or treatment that is already planned are usually excluded – so take out a policy before you actively plan treatment.

How much does orthodontic insurance cost per month?

Typically €15 to €45 per month, depending on your age, the level of cover and the reimbursement rate. A 30-year-old pays around €25–35 per month for a good plan.

Can I take out insurance if I already wear braces?

No. Ongoing treatment is excluded, as are diagnosed misalignments and planned treatment. Once you have a treatment and cost plan from your orthodontist, it is usually too late.

Are there benefit limits in the first years?

Yes, many plans limit reimbursement in the first years, e.g. €1,000 in year 1, €2,000 in year 2, €3,000 in year 3 and unlimited or very high limits afterwards. Premium plans often have higher limits.

Can I choose my orthodontist freely?

Yes, with most plans you can choose any dentist or orthodontist, including private practices.

Can I use ZahnzusatzPlus if I do not live in Germany?

No. We only advise customers who live in Germany and have German health insurance (statutory or private). The plans we compare are German supplementary dental insurance policies, which require a residence in Germany. If you have recently moved to Germany, we are happy to help you.
